Complete the Cycle Safe Video Production application form to apply for your child (ages 10 – 14) to be part of the Cycle Safe Program. You must provide contact information, an up-to-date headshot, and complete the following consent form. Questions can be directed to Fanshawe Professor & Producer of the Video Production Andrew Clark.
